website won't take new brewery

Hello, I've tried to enter a new brewery here in Ohio, called Old Capitol Brewing, everytime I input the information and hit save the screen reverts to being blank and provdies no error message and doesn't save the brewery.
Additionally, how do you add a second site, Fifty West brewery has opened a 2nd loccation where they do brew beer at, including some site specific ones?
